Deschloroetizolam
Deschloroetizolam is a thienotriazolodiazepine and the dechlorinated analog of etizolam.1 It first appeared through grey market research chemical vendors in 2014 and is not prescribed for any medical use. It produces depressant, anxiolytic, sedative, muscle relaxant, and amnesic effects through modulation of GABA receptors. Compared to its parent compound etizolam, it is roughly half as potent but exhibits approximately twice the duration of action.

Paradoxical reactions to benzodiazepines such as increased seizures (in epileptics), aggression, increased anxiety, violent behavior, loss of impulse control, irritability and suicidal behavior sometimes occur (although they are rare in the general population, with an incidence rate below 1%). These paradoxical effects occur with greater frequency in recreational abusers, individuals with mental disorders, children, and patients on high-dosage regimes.

Deschloroetizolam acts as a positive allosteric modulator at GABA-A receptors via the benzodiazepine binding site.2 By enhancing the efficiency of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA) at these receptors, it produces its characteristic depressant and anxiolytic effects.2 It is the dechlorinated analog of etizolam and is reported to be approximately half as potent as that parent compound.2

Limited pharmacokinetic data is available for deschloroetizolam.3 It is reported to have a bioavailability similar to that of its parent compound etizolam.

HighDeschloroetizolam is described as extremely psychologically addictive. Compulsive redosing is commonly reported as a characteristic effect, and the drug is classified as habit-forming.
Physical
Extremely HighDeschloroetizolam produces severe physical dependence with regular use. Sudden discontinuation can be life-threatening, potentially resulting in seizures or death.4 Withdrawal symptoms may occur after a few weeks of steady dosing, and a gradual taper over weeks is strongly recommended rather than abrupt cessation.4
Psychosis Risk
Paradoxical reactions including delusions may occur but are rare in the general population, with an incidence rate below 1%.5 These effects occur with greater frequency in recreational abusers, individuals with mental disorders, children, and patients on high-dosage regimes.5 Delusions may also occur as a symptom of overdose.
Seizure Risk
While deschloroetizolam itself has anticonvulsant properties, paradoxical increased seizures may rarely occur in epileptic individuals. The primary seizure risk is associated with discontinuation after regular use rather than acute administration.4 Drugs which lower the seizure threshold should be avoided during withdrawal.
