Alcohol primarily acts as a central nervous system depressant through positive allosteric modulation of GABA-A receptors, enhancing the inhibitory effects of gamma-aminobutyric acid in the brain.47 It simultaneously functions as an antagonist at NMDA glutamate receptors, reducing excitatory neurotransmission.4 Alcohol also acts as an agonist at glycine receptors and inhibits voltage-dependent L-type calcium channels.4 The substance directly affects multiple additional neurotransmitter systems including those of acetylcholine and serotonin.78 The pleasurable effects of alcohol ingestion result from increased levels of dopamine and endogenous opioids in reward pathways.9
Pharmacokinetics
Alcohol is rapidly absorbed after oral ingestion via the stomach and intestines into the bloodstream.10 It is highly water-soluble and diffuses passively throughout the body, including the brain.10 Approximately 90% or more is metabolized hepatically, primarily by the enzyme alcohol dehydrogenase and cytochrome P450 enzyme CYP2E1.11 One standard drink is sufficient to nearly saturate the liver's metabolic capacity.12 The remaining 5 to 10% is excreted unchanged in breath, urine, and sweat.11

Retail sale and consumption are governed by applicable provincial legislation: the Act defines a retail seller as one registered or licensed under that provincial law, and the Constitution places liquor licences in Schedule 5 provincial competence. The Act also recognises a private collection acquired for private consumption without intent to resell.

The law regulates commercial turnover and retail supply rather than creating a general ban on adult possession or consumption.

The law prohibits sale of every type of alcoholic beverage to persons under 18 nationwide, and defines alcoholic beverages as those containing alcohol regardless of strength. It also prohibits consumption in public streets and inside stadiums or other venues during mass sporting, educational, cultural, or artistic activities, except at places and times expressly authorised by the competent authority; it does not establish a general adult-possession offence.

Since 1 July 2024, the law also prohibits sales from vending machines, restricts petrol-station sales overnight, and limits sales in hospitals.

The Public Health (Alcohol) Act 2018 additionally regulates minimum pricing, labelling, advertising, and aspects of sale and supply of alcohol products.

Malaysia regulates retail sales of intoxicating liquor under section 32 of the Excise Act 1976. A person may not sell or offer intoxicating liquor for retail sale except under, and in accordance with, a licence issued under Part IV and at the place specified by that licence. Section 32 provides specified exceptions, including certain sealed beer or toddy products and specified duty-free sales; this federal provision regulates retail sale rather than creating a general national consumption ban.

Alcohol is lawful but regulated under Norway's Alcohol Act. Section 1-4a requires a licence for sale, serving, and manufacture of alcoholic beverages; the Act distinguishes off-premises sale from serving for consumption on the premises. Section 1-5 prohibits sale, serving, or delivery of group 3 alcohol to persons under 20 and of alcohol-weak drinks and groups 1 and 2 to persons under 18. The statutory restriction is directed at the seller, server, or deliverer rather than creating a general adult-possession prohibition.

Portugal regulates the availability, sale and consumption of alcoholic beverages in public places and places open to the public under Decreto-Lei n.º 50/2013. Article 3 prohibits supplying, selling or making alcoholic beverages available to minors, and the 2015 amendment applies the prohibition to all persons under 18. The regime also restricts consumption by minors in the covered places and provides administrative-offence enforcement.

The Act requires a liquor licence for supply at premises and regulates trading hours; it also establishes restrictions on public consumption and Liquor Control Zones. The statutory definition covers beverages or mixtures containing more than 0.5% ethanol by mass or volume.
