Flurazepam has significant abuse potential with risk of psychological dependence. Recreational misuse occurs where the drug is taken to achieve a high, and continued use against medical advice is common. The FDA required a boxed warning describing risks of abuse, misuse, and addiction for all benzodiazepines.

Physical

High

Long-term use is associated with drug tolerance and physical dependence. After discontinuation, a rebound effect or benzodiazepine withdrawal syndrome may occur approximately four days after stopping medication. Flurazepam shares cross tolerance with barbiturates, indicating significant physical dependence potential.

Toxicity

Lethal Dosage

Flurazepam combined with alcohol or other CNS depressants may produce fatal results. Overdose symptoms include coma, confusion, low blood pressure, and severe sleepiness.

Central Nervous System

Chronic use causes cognitive impairment including anterograde amnesia, daytime sedation, and motor incoordination; these effects may persist as residual hangover effects after nighttime administration and impair next-day functioning.

Musculoskeletal

Use is associated with impairments in body balance and standing steadiness, leading to increased risk of falls and hip fractures, particularly in individuals who wake at night or the next morning.

Immune

In a meta-analysis, people using hypnotic drugs had 44% more mild infections, including examples such as pharyngitis and sinusitis, than those receiving placebo.

Psychosis Risk

Paradoxical agitation or disinhibition may occur, particularly in elderly patients, though this is considered a moderately higher risk compared to non-benzodiazepine sedatives rather than a common occurrence.
