Lithium is an alkali-metal monovalent cation typically administered as carbonate or citrate salts.citation needed Classified as both a mood stabilizer and antidepressant, it is primarily used in the management of mood disorders. Lithium does not produce an acute recreational or psychedelic experience; its subjective effects emerge gradually over days to weeks of consistent dosing. The defining feature is mood stabilization, in which the extreme highs and lows of bipolar disorder are dampened toward a more even emotional baseline, alongside antidepressant effects. Many users describe the state as emotionally quieter rather than euphoric, and some report a subtle dulling of affect or cognition at therapeutic levels.

Physical

Physical effects at therapeutic doses commonly include a fine hand tremor, increased thirst and urination, mild nausea, and fatigue or sedation, especially early in treatment.

Side effects are most prominent when starting treatment or at higher blood concentrations.

Cognitive

The headspace is characterized by a flattening of emotional extremes and a steadier, less reactive mood. Some individuals experience mild cognitive slowing or a sense of emotional blunting, particularly at higher therapeutic concentrations.

Lithium's biological effects are thought to stem primarily from the ability of the monovalent lithium ion (Li⁺) to compete with endogenous ions, particularly sodium (Na⁺) and magnesium (Mg²⁺), whose ionic radius (86 pm) closely approximates that of lithium (90 pm).citation needed Lithium enters cells through sodium transport mechanisms, including both voltage-gated and epithelial sodium channels as well as ligand-gated ion channels, and can cross nuclear and mitochondrial membranes. It can also enter and partially block potassium and calcium channels without permeating them. The full mechanisms underlying lithium's mood-stabilizing and antidepressant effects remain only partially understood.1

For an acute manic episode, the cited U.S. label says lithium may normalize symptoms within 1–3 weeks.2 A general NHS patient guide instead says that lithium may take several weeks or months to work; these two timelines have different scopes and should not be averaged.citation needed Neither is equivalent to recreational-drug onset, come-up, peak, offset, or total duration, and maintenance treatment continues according to the prescribing plan.

Pharmacokinetics

Oral lithium is reported to be completely absorbed in the upper gastrointestinal tract. Peak serum concentrations occur 0.25–3 hours after immediate-release preparations and 2–6 hours after sustained-release preparations. Its distribution approximates total body water, plasma-protein binding is negligible, and the apparent volume of distribution at equilibrium is 0.7–1 L/kg. Lithium is not metabolized. It is primarily excreted in urine; approximately 80% of filtered lithium is reabsorbed in the proximal tubule, and the elimination half-life is approximately 18–36 hours.3

Routine progressive tolerance to lithium's maintenance effect is not established. In a prospective naturalistic cohort of 346 people with bipolar I or II disorder followed for a mean of 10 years (range 1–20), the morbidity index remained stable and was not associated with treatment duration, although the study had no comparison group and substantial attrition.4 Some early adverse effects, including mild nausea and general discomfort, may subside with continued treatment; this is side-effect adaptation, not a reason to increase the dose.citation needed Breakthrough symptoms require clinical review and serum-level assessment.

International

Lithium is not named in the schedules to the 1961 Single Convention or the 1971 Convention on Psychotropic Substances. It is also not listed in the 1988 Convention tables of controlled precursors. Lithium is therefore not internationally scheduled under these three conventions.
