ModerateWhile seemingly less compulsive than related cathinones like α-PHP and MDPV, users can still become trapped in cycles of chasing progressively shorter-lasting euphoria and avoiding the comedown. Some users report compulsive redosing patterns despite the drug not being particularly euphoric, with the short-lasting positive effects and undesirable comedown driving continued use.

No fatalities have been reported, although fatal toxicity is believed possible with overdose. One case of fetal death was temporally linked to MDPHP and α-PHP use in a pregnant female, though the exact cause of the intrauterine asphyxia was not determined.
Cardiovascular
Increased heart rate and blood pressure occur commonly during use; overdose could cause excessively elevated cardiovascular parameters, with complications such as myocardial infarction and stroke considered possible though not yet documented in medical case reports.
Psychosis Risk
Stimulant psychosis is possible with high doses, prolonged use, and sleep deprivation, but is not the typical effect at reasonable doses with adequate sleep. Case reports have documented psychosis and aggressiveness requiring police assistance. Nearly all severe experiences reported involve prolonged use over 1-2 days with little to no sleep.
