Classified as having low potential for abuse, with no indication of script-chasing or doctor-shopping detected among prescribed patients. However, patients with a history of substance abuse or dependence may be more likely to abuse dronabinol.

Moderate

Physical dependence can develop during chronic therapy. A withdrawal syndrome was reported after abrupt discontinuation in subjects receiving 210 mg per day for 12 to 16 consecutive days. Withdrawal symptoms include irritability, insomnia, restlessness, hot flashes, sweating, rhinorrhea, loose stools, hiccoughs, and anorexia, typically resolving over 48-96 hours. Disturbed sleep may persist for several weeks after discontinuing high-dose therapy.

Reproductive/Developmental

Administration during pregnancy may cause fetal harm, including decreased maternal weight gain, reduced viable pups, increased fetal mortality, and early resorptions at doses inducing maternal toxicity in animal studies.

Central Nervous System

Reversible effects on cognition, memory, and perception occur during use; pediatric patients may be more sensitive to neurological and psychoactive effects.

Cardiovascular

Sympathomimetic activity may result in tachycardia and conjunctival injection during use; orthostatic hypotension and syncope upon abrupt standing have been reported.

Psychosis Risk

Psychiatric adverse reactions may include psychosis, hallucinations, depersonalization, mood changes, and paranoia. High doses, abuse, or misuse increase risk. Panic reactions may be more likely in patients with a history of anxiety or nervousness.

Seizure Risk

Patients who already have seizure disorders may experience seizures, especially during overdose.
