SAM-e

On 14 May 2026, Lithuania's Institute of Hygiene concluded under Article 4 of Regulation (EU) 2015/2283 that S-adenosyl-L-methionine disulfate tosylate, also identified as S-adenosyl-L-methionine disulfate p-toluenesulfonate, is a novel food because significant consumption of that salt as food or a food supplement in the Union before 15 May 1997 was not demonstrated; the determination does not cover S-adenosyl-L-methionine as such or every product described as SAM-e. In Northern Ireland, Article 6 permits a novel food to be placed on the market or used in food only when it is authorised and included in the Union list, which contains no authorisation for this salt under either exact name in its updates through 20 July 2026. In Great Britain, assimilated Article 6 similarly requires authorisation by the appropriate authority, and the Food Standards Agency register updated 31 March 2026 contains no entry for S-adenosyl-L-methionine, SAM-e, disulfate tosylate, or disulfate p-toluenesulfonate; Lithuania's 2026 determination is not binding in Great Britain, and a binding determination that the salt is novel there would require a domestic Article 4 decision. Commission Implementing Regulation (EU) 2019/921 assigns CN 2106 90 92 to the specifically described tablet containing 400 mg of this salt when presented in bulk as a food supplement, but that customs classification does not grant novel-food authorisation.

Ademetionin, the German INN spelling of ademetionine, is expressly listed in Anlage 1 of the Arzneimittelverschreibungsverordnung (AMVV). Under § 1 AMVV, medicinal products consisting of, prepared from, or containing ademetionine may be dispensed only on a medical, dental, or veterinary prescription, unless another AMVV provision applies. This rule applies only when the particular product is classified as a medicinal product under § 2 of the Arzneimittelgesetz; classification is determined case by case from the product's composition, dosage form, packaging, labelling, leaflet, manufacturer-attributable information, and advertising. In judgment 13 A 2432/18, the Higher Administrative Court of North Rhine-Westphalia held that the specific capsules before it, containing 200 mg S-Adenosylmethionin and 200 mg magnesium citrate at one capsule per day and marketed as a food supplement, were neither functional nor presentation medicinal products on the facts and law as of 4 July 2016.

SAM-e is sold in the United States as a dietary supplement, but federal law classifies the finished product rather than treating SAM-e as a dietary supplement in every use. A qualifying product must meet the ingredient, intended-ingestion, presentation, and labeling conditions in 21 U.S.C. § 321(ff). FDA does not approve dietary supplements before marketing; manufacturers and distributors remain responsible for safety, labeling, and current good manufacturing practices. For a product containing a new dietary ingredient that is not exempt under 21 U.S.C. § 350b(a)(1), the manufacturer or distributor must provide the Secretary with the statutory safety information at least 75 days before introduction or delivery for introduction into interstate commerce. FDA's inventory records SAM-e-related new dietary ingredient notifications, but those notifications are not premarket approvals. This federal framework does not establish that every SAM-e source, salt, formulation, dose, claim, route of administration, or finished product is lawful, and it does not determine state, district, territorial, tribal, or local law.

Under regulations 7 to 9 of the Control of Drugs and Cosmetics Regulations 1984, a product generally must be registered and a person who manufactures, sells, supplies, imports, possesses, or administers it must hold the appropriate licence. The Drug Control Authority's Meeting No. 408 approved-products list classifies Medispec SAM-e 400 mg Enteric Coated Tablet as Health Supplement—General under registration number MAL25046050NC, with registration from 29 April 2025 through 29 April 2030. This approval is product-specific and does not authorize every SAM-e formulation or import.

In Russia, the official State Register identifies ademetionine, the medicinal name for S-adenosyl-L-methionine, as the active substance in two registered Heptral 400 mg presentations. The enteric-coated tablets in packs of 10, 20, and 40 are dispensed without a prescription, while the lyophilizate for intravenous or intramuscular injection in five-vial packs is dispensed by prescription.
