Nutmeg is a common culinary spice derived from the seeds of Myristica fragrans,citation needed a plant indigenous to Indonesia and New Guinea. Its primary psychoactive constituent is myristicin,1 a phenylpropene aromatic ether.citation needed When consumed orally in quantities far exceeding culinary use, nutmeg produces deliriant, hallucinogenic, and oneirogenic effects. Nutmeg produces a delirious, dysphoric intoxication that is widely regarded as unpleasant rather than recreational. Onset is unusually delayed, often taking several hours before effects appear, which frequently leads users to believe the dose was inactive. Once present, the experience is dominated by confusion, nausea, dizziness, and severe physical discomfort rather than any coherent psychedelic state, and the aftermath can extend into a hangover of dry mouth and general illness lasting a day or more, with lingering malaise sometimes persisting for several days.

The body load is heavy and punishing, featuring nausea, sickening dizziness, severe headache, and motor impairment severe enough to prevent standing or walking. Pronounced dry mouth and a prolonged, debilitating hangover follow the acute effects.

The psychoactive effects of nutmeg appear to derive from anticholinergic-like hallucinogenic mechanisms attributed primarily to myristicin and elemicin, two phenylpropanoid compounds present in the essential oil.citation needed Myristicin has additionally been identified as a monoamine oxidase inhibitor.12 The essential oil is a complex mixture containing dozens of terpenes and phenylpropanoids, and the overall psychoactive profile likely reflects contributions from multiple constituents rather than a single compound.3

Pharmacokinetics

Whole-nutmeg pharmacokinetics cannot be represented by one constituent. In a single 75 g poisoning case, serial serum measurements were used to estimate half-lives of 19 hours for myristicin, 17 hours for safrole, and 8.5 hours for elemicin. [S1][S2] Human urinary analysis after suspected nutmeg abuse identified metabolites of elemicin, myristicin, and safrole; neither the parent constituents nor the previously proposed amphetamine derivatives were detected in that urine sample.
