4-EMC is a synthetic stimulant and entactogen of the substituted cathinone class.citation needed It is a structural isomer of both 4-MEC and 3,4-DMMC. Initially identified in Europe, it has since been detected in numerous countries worldwide, first appearing in Australia in 2020. As a designer drug with limited research history, there is minimal documentation regarding its safety profile and long-term effects, and it is considered habit-forming.

The experience is that of a euphoric, socially disinhibiting stimulant, with users reporting mood lift, increased energy and alertness, heightened sociability, and increased libido. Consistent with other substituted cathinones, the effects center on stimulation and emotional elevation rather than pronounced sensory alteration, although visual and auditory hallucinations have been reported. Less pleasant emotional shifts such as moodiness and aggressiveness can also occur, particularly as the experience wears on.

Pharmacology

Pharmacodynamics

In HEK293 cells expressing human monoamine transporters, 4-EMC inhibited norepinephrine, serotonin, and dopamine uptake with IC50 values of 2.5 µM (95% CI 1.7–3.7), 4.3 µM (3.2–5.9), and 31 µM (13–72), respectively. In this assay it was more potent at NET and SERT than at DAT.1

4-EMC also induced norepinephrine and dopamine efflux and produced serotonin release through SERT, described as similar to MDMA in the same cell system. These in-vitro results do not establish human potency, dose, safety, metabolism, or pharmacokinetics.1
